Job Title: RN Case Manager

Pay Rate: $2000.00 - $2700.00

Position Summary: The RN Case Manager will coordinate all post-acute needs for patients.

Minimum Requirements:

• Acute care case management/discharge planning experience
• Experience with Interqual/Milliman
• Familiarity with the California market
• Previous HCA case management experience
• At least 2 years of experience

Assignment Length: 13 weeks

Shift: Day Shift, 8am - 4:30 PM

Systems Used: Meditech and Midas
