About the Opportunity:

This is a fulfilling chance for a registered nurse to play a vital role in coordinating care and advocating for children with special needs within a dynamic and supportive environment. You will work collaboratively with a team of healthcare professionals to ensure these children receive the comprehensive care and resources they deserve.

Responsibilities:

• Conduct comprehensive assessments to identify the unique needs of each child with special needs.
• Develop and implement individualized care plans in collaboration with patients, families, physicians, and other healthcare providers.
• Coordinate and manage care across various departments, including inpatient, outpatient, and community-based services.
• Educate and empower patients, families, and caregivers on managing special needs.
• Advocate for children with special needs and ensure they have access to necessary resources and support systems.
• Monitor and track patient progress, documenting care interventions and outcomes.
• Maintain accurate and detailed case management records.
• Participate in quality improvement initiatives to enhance the care provided to children with special needs.

Qualifications:

• Active and unrestricted Pennsylvania Registered Nurse (RN) license.
• Minimum 2 years of experience in pediatrics or a related field, with a focus on special needs preferred.
• Strong understanding of special needs conditions and best practices in care management.
• Excellent communication, collaboration, and interpersonal skills.
• Compassionate and patient-centered approach to care.
• Ability to work independently and as part of a team.
• Exceptional organizational and time management skills.
• Proficient in electronic health record (EHR) systems.
