RN Case Manager - Sign On Bonus. - White Plains Hospital.

Position Summary:
The RN Care Manager conducts comprehensive assessments upon patient admission, identifying high-risk indicators to ensure appropriate supportive services and discharge planning, thereby preventing readmissions. Ongoing reassessment of patient needs is performed throughout the hospital stay, along with utilization review (UR) activities as per departmental policy.

Essential Functions:
- Adhere to performance standards and complete initial assessments.
- Coordinate multidisciplinary care and transitional planning activities.
- Educate patients/caregivers and arrange aftercare.
- Communicate relevant patient information to the healthcare team.
- Document according to departmental requirements and participate in performance improvement activities.
- Conduct UR activities including payer reviews and clinical assessments.

Education & Experience:
- BSN or related Bachelor's degree required.
- 5 years of clinical experience in ED, critical care, or med/surg.
- Previous case management experience preferred.
- NYS RN license required; certifications in PRI/Screen and C.C.M. preferred.

- Full Time, Day, Mon-Fri, with a monthly weekend rotation.
