Position Overview

We are seeking a PRN (as-needed) Registered Nurse (RN) Case Manager to join our dynamic team. As a Case Manager, you will play a pivotal role in coordinating and delivering comprehensive hospice care to our patients. Your compassion, clinical expertise, and communication skills will make a significant impact on the lives of those we serve.
 

Job Qualifications

Education: Must be a graduate of an accredited Nursing School

Licensure: Current state license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license.

Experience: Two (2) years experience as a registered nurse in a clinical care setting. Hospice experience preferred.

Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write and comprehend English.

Transportation: Reliable transportation and valid auto liability insurance.

Responsibilities

• Conduct initial assessments and ongoing evaluations of patients’ needs
• Develop and implement individualized care plans
• Collaborate with interdisciplinary teams to ensure holistic care
• Provide education and emotional support to patients and families
• Coordinate services, including home visits and referrals
• Maintain accurate documentation and records
• Participate in quality improvement initiatives
