Key Responsibilities:

Promote compassionate care for patients with life-limiting illnesses.

Conduct assessments and develop care plans in collaboration with the care team.

Educate patients and families on care needs and hospice philosophy.

Maintain communication with families and healthcare providers.

Ensure compliance with care standards and documentation.

Requirements:

Current RN license and one year of relevant experience.
