Case Manager Description/Duties

• Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process. Performs utilization management and quality screenings

Case Manager Requirements

• Active RN License for the State of NJ (must be in good standing)
• 2 years of acute care experience
• In-patient Case Management required and Managed Care preferred
• BSN Required
