Pay Rate: $2000.00 - $2700.00

Position Summary: The RN Case Manager will focus on care coordination, continuum of care, discharge planning, and developing patient care plans.

Minimum Qualifications:

• FL/compact RN license
• 1+ year of acute care hospital case management experience

Assignment Length: 13 weeks

Shift: 8:00 AM - 4:30 PM, 40 hours/week

Systems Used: Meditech, Midas, Wellsky
