The RN Case Manager - Infectious Disease in Lynn, MA will provide comprehensive care management for patients with infectious diseases. This role involves coordinating treatment plans, educating patients, and collaborating with healthcare teams to ensure optimal patient outcomes.Responsibilities:

Assess patient needs and develop individualized care plans

Coordinate with healthcare providers and community resources

Requirements:

Current RN license in Massachusetts

Bachelor's degree in nursing or related field
