Job Title: RN Case Manager Hospice

Location: Scottdale, PA

Salary: $85,000 - $90,000 per year

We are seeking a compassionate RN Case Manager for our hospice team. Responsibilities include:

Conducting patient assessments and developing care plans

Coordinating with interdisciplinary teams to ensure quality care

Providing education and support to patients and families

Monitoring patient progress and adjusting care plans as needed

Documenting patient care and maintaining accurate records

Qualifications:

Current RN license in Pennsylvania

Experience in hospice or palliative care preferred

Strong communication and organizational skills
