We are seeking a full-time Registered Nurse to join our growing Hospice Team, dedicated to providing high-quality care to patients and their families during end-of-life transitions. Qualifications:

• Graduate of an accredited nursing program
• Current RN license
• One year of clinical experience required; hospice/home health experience preferred
• Reliable transportation and valid driver’s license

Essential Functions:

• Conduct assessments and develop care plans
• Monitor patient needs and coordinate services
• Provide nursing care and educate patients and families
• Document clinical notes and participate in professional development
