Job Details
RN Case Manager - Home Health
- Mon – Fri, Day Shift
- Seeking a Registered Nurse to provide in-home patient care
- Provide case management, OASIS admissions, wound care, medication administration, etc.
Qualifications:
- NEW GRAD RN’s Welcome
- Experience with OASIS, wound care, 485’s preferred but not required
- Strong assessment, critical thinking, judgement and decision making
- Active NY State license
- Drivers license in good standing
- Bilingual a plus
