About the Job
ChristianaCare is seeking to hire a full-time RN Case Manager for our Heart and Vascular Center in Newark, DE. The selected candidate will serve as an inpatient nurse navigator to support on-site heart failure patient education and discharge planning. The Case Manager will also support the heart failure multi-disciplinary team to maintain heart failure accreditation core measures through performing 72 hour virtual follow up phone calls on all patients discharged with a primary diagnosis of heart failure.
Work Schedule
Monday-Friday, day shift
No weekends or holidays
Principal Duties and Responsibilities
- Perform ongoing telephonic and/or embedded activities of assessment, problem identification, planning, implementation, coordination, monitoring, and evaluation of patients discharged with a primary heart failure diagnosis
- Collaborates with patients, caregivers, and members of the multi-disciplinary care team to develop a patientcenteredplan of care to meetidentifiedpatientcaregoalsand outcomes
- Identifies issues that serve as barriers to care. Participates in development and implementation of strategies to remove barriers and promote resolution through coordination of a problem-solvingprocess
- Promote patient wellness and autonomy through advocacy, communication, education, and identification and referral to community resources or other case management programs such as disease managementprograms
- Collaborate with healthcare professionals throughout the care continuum to facilitate the transition of the patient to the appropriatelevel of care
- Demonstrates knowledge of utilization management and care coordination processes and current standards of care as a foundation for transition planning, complex case management and disease managementactivities
- Confers with Medical Directors regarding complex cases requiring physician input regarding the treatment plan or physician to treating physiciancollaboration
- Receives Super-user training on a variety of programs, IT software, educational presentations, and work with staff in a Super-user capacity to ensure staffcompliance.
- Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violationsthat may causeinjury to oneself,otheremployees,membersandvisitors
- Performs all other related duties as assigned by Manager/Director.
Education and Experience Requirements
- Graduate of an accredited school of nursing; BSN within 3 years of hire
- Masters degree preferred.
- Minimum of 1 -2 years of clinical experience in acute care.
- Case Management or utilization management experience desirable.
- Prior cardiology/heart failure experience preferred
Special Requirements
- Active Clinical RN License required.
- Case Management Certification (CCM) required within 18 months of eligibility.
