If that describes you...you are made for this!The RN Case Manager will be based out of our Portsmouth, NH office and will service the beautiful Greater Seacoast community.The RN Case Manager is responsible for managing a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community. Open positions include Full Time, Part Time, and Per Diem.

Pay:$36-$40/hour

Hours:Monday-Friday 8:30am-5pm(Flexible days if desired!)

Location:Greater Seacoast Area

Job Requirementsof theHome Health Registered Nurse Case Manager:

• Valid Nursing License in the State of New Hampshire
• Nursing experience required
• Current CPR Certification required (We offer monthly renewal classes paid for by Interim HealthCare)
• Reliable transportation

*A Sign on Bonus of $5,000 is offered to RN Case Managers that have two years of current nursing experience working at least 30 hours per week. After 90 days of employment 20% of the sign on bonus will be paid out. The remaining sign on bonus will be paid after 6 months of employment.
