Job Summary: The RN Case Manager in the Emergency Department will coordinate patient care, ensuring efficient and effective treatment plans.

Key Responsibilities:

Assess patient needs and develop care plans

Collaborate with healthcare teams for optimal patient outcomes

Facilitate discharge planning and follow-up care

Monitor patient progress and adjust care plans as necessary

Educate patients and families on health management

Qualifications:

Current RN license in PA

Experience in emergency care preferred
