RN Case Manager - Days - Integris Health

A leading not-for-profit health system in Oklahoma City is seeking a part-time RN Case Manager to join their Case Management team. The role involves working from 8 a.m. to 4:30 p.m. and focuses on managing patient care, ensuring optimal clinical and financial outcomes, and facilitating efficient patient movement throughout the continuum of care.

Key responsibilities include:

• Conducting comprehensive assessments of patients' needs.
• Coordinating timely transfers and discharge planning.
• Communicating care plans to patients and healthcare professionals.
• Monitoring resource utilization and compliance with regulations.
• Collaborating with various departments to ensure effective patient care.

Applicants should have 5 years of clinical experience, current RN licensure in Oklahoma, and strong communication skills. A BSN and Case Management Certification are preferred.
