RN Case Manager - UniversaCare

Position: RN Case Manager – Per-Diem

Location: Waterbury, CT and surrounding towns.

Education/Qualifications: Candidates must possess a current Connecticut RN license and possess and maintain CPR certification. Home health care experience is preferred.

Shift/Schedule: Days, evenings, and weekend. 

Benefits:

1. Competitive Pay –

a. Paid on a per-diem visit for visits completed on a weekly basis, at the applicable per diem rates. 

b. Orientation hours are paid hourly at $25.00/hour, until the RN is signed off to work independently.

2. Mileage reimbursement of $0.50/mile.

Job Description:

• Makes the initial evaluation/assessment visit.

• Initiates and revises a plan of care for assigned patients and family members.

• Completes OASIS assessments at appropriate time points.

• Regularly re-evaluates the patient's nursing needs.

• Prepares clinical and progress notes. Coordinates services.

• Makes referrals to other services as needed.

• Coordinates and participates in the patient’s discharge planning.

• Must comply with accepted professional standards and practices.

• Demonstrates good verbal and written communication and organizational skills.

• Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
