Job Details
- Perform clinical assessments and screenings for home care clients.
- Visit long-term care facilities to assess discharge potential and complete necessary documentation.
- Develop and reassess care plans for clients referred for personal care.
- Provide consultation on medical and nursing issues to case managers and staff.
- Collaborate with peers to promote cost-effective service plans and determine program eligibility.
- Participate in waiver programs and screening for enhanced services.
- Maintain collaborative relationships with agencies and institutions serving our clients.
Skills You Need:
- Valid Massachusetts RN license with clinical experience in community or long-term care settings.
- Knowledge of home care systems and community resources.
- Effective communication and collaboration skills.
- Proficiency in internet, email, and computer usage.
