Job Title: RN Case Manager

Pay Rate: $2000.00 - $2700.00

Position Summary: The RN Case Manager will focus on discharge planning, conducting in-depth interviews with high-risk patients, and collaborating with an interdisciplinary team.

Minimum Qualifications:

• 2+ years in an acute inpatient setting (first-time travelers with acute care case management experience considered)
• RN license; BLS certification

Preferred Qualifications: Epic experience; CCM or ACM certification.

Assignment Length: 13 weeks

Shift: 7:30am-4pm, 40 hours/week
