Qualifications
Required:
- Associate degree or higher
- Wound Care (4+ years)
- RN (4+ years)
- 4+ years of experience in ANY of the following:
- Geriatric Care
- Senior Care
- Geriatric/LTC
- Valid American Heart Association Basic Life Support (AHA BLS)
- Valid Registered Nurse (RN) in the state of California (CA)
- Bilingual in Spanish
- Looking for a full-time position
Preferred:
- Valid Alzheimer's certification
- Experience working for a PACE (Program of All-Inclusive Care for the Elderly) model
Overview: In this role, you will be responsible for managing and delivering direct nursing care to participants of our Award Winning Client's PACE program. Our client, a nationally recognized organization with numerous accolades, operates with a commitment to excellence in elder care.
Key Responsibilities:
- Contribute to patient assessments, care plans, delegation, and service determination
- Lead personalized care for participants, coordinating with medical providers
- Deliver care in various settings, including homes, clinics, and SNFs
- Conduct comprehensive nursing assessments
- Initiate therapies within scope of practice in emergency situations
- Educate participants, caregivers, and staff on care plans
- Monitor and evaluate therapeutic interventions
Schedule and Shift Details:
- 40 hours per week, Monday-Friday, with occasional on-call coverage
- Schedule may vary

Attributes of an Ideal Candidate:
- Passion for serving high-risk seniors and frail older adults
- Minimum 2 years of relevant experience
- Functions independently in clinical management
- Active listening and conflict management skills
- Training in wound care, IVs, blood draws
- Active, unrestricted RN license in California
- Graduate of an accredited School of Nursing (BSN preferred)
- CPR and First Aid certification (within 60 days of employment)
- Alzheimer's certification
- Languages Spoken: English and Spanish required
- Ability to lift and carry up to 50 pounds
- Assist Participants with mobility challenges
