RN Case Manager Admissions - Hospice - Days. - Integris Health.

We have an exciting opportunity for a full-time RN Case Manager in Oklahoma City, OK, working with our Hospice team from 8a-5p. This role involves providing exceptional care to hospice patients, managing service delivery per physician orders, and participating in interdisciplinary meetings. Responsibilities include supervising Home Health Aide activities, educating nursing students, and specializing in pain and symptom management. Candidates must have an Associate's or Bachelor's degree in nursing, current RN licensure in Oklahoma, BLS certification, and at least one year of clinical nursing experience, preferably in Hospice. A valid Oklahoma driver's license is required.
