REGISTERED NURSE RN CARE MANAGER - HOME HEALTH SERVICES

While the main service area is Marietta, GA, should the need arise, you may have to support a nearby area. 

 We offer a Salary Plus pay model which means, you earn a generous base salary, and should you go over productivity (which is very likely), you will earn additional money on top of your base salary.  While home health experience is preferred, we will gladly and properly train a RN with the heart for home health.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a home care setting

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS
