PRIMARY PURPOSE:

The RN Care Coordinator Team Leader is responsible for coaching associates in the Ambulatory Care Management department under the supervision of a Manager of Care Coordination. This includes associate supervision and the achievement of organizational goals and objectives. The RN Care Coordinator Team Leader is a highly visible position that is responsible for creating positive associate engagement, as well as successful patient experiences, both in person and telephonically. The RN Care Coordinator Team Leader is a first-level exempt designed to meet business needs as well as create a career development opportunity in preparation for broader leadership roles.

EDUCATION:

Minimum: Bachelor’s Degree and active RN Licensure

YEARS OF EXPERIENCE:

Minimum: 3+ years’ experience as a nurse.

Desired: 1+ years’ supervisor/coaching experience

REQUIRED SKILLS AND KNOWLEDGE:

Ability to manage and prioritize multiple tasks, knowledge of EPIC, Microsoft Excel, Word, Outlook and PowerPoint and the ability to learn other computer skills. Must have good organizational skills and work professionally with providers, hospital administration and management, SEP employees and the public.

OTHER REQUIRED SKILLS AND KNOWLEDGE:

1. Knowledge of current Care Management workflow and process.
2. Demonstrated ability to work successfully in a team-based, decision-making culture.
3. Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
4. Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.
5. Must respond to and follow through with requests from patients and/or stakeholders promptly.
6. Must work carefully and precisely with attention to detail.
7. Must utilize resources wisely.
8. Performs duties willingly and with initiative. Shares necessary information so co-workers can do the same.
9. Cooperates with other departments and work groups.

LICENSES AND CERTIFICATIONS:

Minimum: Active RN Licensure; Must have reliable transportation for local travel between work locations.
