RN Care Coordinator - Hospice. - Chapters Health System.

Qualifications:

• Current RN license in the state of employment
• 1 year of inpatient nursing or hospice experience (external candidates); 6 months LPN experience (internal candidates)
• Experience with EMR/EHR systems
• Valid driver's license and reliable transportation
• Ability to manage emotional stress and physical demands of the role
• Active BLS certification required
