Responsibilities:

• Accurately document all information pertaining to each resident’s performance in their respective restorative program on the daily flow sheets and in weekly/monthly summary.
• Implement nursing rehabilitation’s restorative interventions as directed by the therapists, charge nurse, and/or the director of nursing, including bed mobility, transfers, walking, communication, splint or brace assistance, passive and active range of motion, training, and skill practice.
• Provide care services to the residents, within the framework of the resident’s activities of daily living and functional abilities.
• Assist with transfers, walking, and range of motion exercises
• Ensure residents have clean, well-maintained clothing and linens
• Report any significant changes in residents' conditions to the appropriate personnel
• Other duties as assigned by the supervisor.

Qualifications:

• Valid State CNA and  RNA certification
• CPR or BLS Certification
• Recent Physical and TB test result (within one year)
• Valid Driver's License or State ID
• Minimum 6 months RNA experience
• 1 year skilled nursing facility experience preferred
