Responsibilities and Duties
- Coordinates all necessary team meetings for the program.
- Develops, modifies, and/or recommends necessary amendments of the service plans to Support Coordinators/Case Managers.
- Demonstrates key knowledge of Division of Developmental Disabilities regulations, circulars, and licensing standards.
- Maintains service programs compliance with DDD, DHS, and other relevant state departments as well as federal rules and regulations.
- Leads and guides program staff to assess clients' strengths and needs in relevant development areas through observation and assessment tools, and in accordance with APluscare guidelines.
- Supervise support and service program supervisors to facilitate desired positive outcome for clients served.
- Promote and model appropriate support system and activities for clients served.
- Maintains service programs full adherence to the Policies and Procedures of APluscare.
- Maintains service programs full adherence to relevant service programs manuals.
- Conducts weekly program visits and supervision with program supervisors.
- Conducts weekly, monthly, and quarterly program audits to ensure quality supports and services for clients.
- Develops, distributes as appropriate, and monitor staff schedule.
- Coordinates the planning and implementation of care and support plans, and assuming the responsibility for the implementation and appropriate documentation as needed.
- Provides the Program Director with regular updates and reports on all clients.
- Prepares and submits monthly reports to the Program Director.
Qualifications and Skills
- A bachelor's degree
- Employee must cooperate with the licensee and department staff in any inspection or investigation.
- Employee must successfully complete and demonstrate proficiency in all areas of required training.
- Valid Driver's License Must be at least 18 years of age
- Push, pull, and lift up to 50lbs
