About the Job
The Residential Coordinator is responsible for supervision of Direct Support Professional (DSP) as they provide support within the home setting. This position represents Impact Community Services in various external settings and directly influences the public's perception of the organization. The ideal candidate for this position will demonstrate excellent communication skills, possess keen attention to detail and experience in sponsor residential programs.
Essential Job Functions*
- Manage DSP in the residential setting to ensure their competent knowledge of relevant Policy and Procedures.
- Assist with providing training to all DSP.
- Conduct visits to DSP's home and complete quality assurance checklists as scheduled.
- Monitors programs and makes suggestions to the team (as needed);
- Ensures implementation of Individual Service Plan (ISP)/Plan for Support (PFS) and quarterly reviews are followed.
- Investigates individual and staff incidents and completes required documentation and ensures compliance with individuals' human rights.
- Participates in discharge planning; monitors behavioral management.
- Scheduled on-call availability for any situation that may occur at Impact Community Services, LLC to include, but not limited to: Crisis Emergencies, Employee Sickness, Vacations, Terminations, Emergency Preparedness Objectives, etc.
- Serves to assist with ensuring that Individuals receiving Services, as well as Employees understand the process of exercising their Rights and resolve complaints regarding Services
In conjunction with the Service and Program Director; the Residential Coordinator:, (1) reviews all activities and documentation to ensure that it is completed in an effective, ethical, and timely manner; (2) Meets with Individuals receiving services and/or Authorized Representatives to ensure a thorough understanding of Impact Community Services' Programs and Services; (3) Provides training, guidance, and support in developing ISP/PFS, Behavior Treatment Plans (BTP), Discharge Plans. (4) Provides training, support and evaluates Impact Community Services, LLC structured program of care with respect to the Provision of Activities of Daily Living (ADLs), Behavioral Management Programs, and Independent Living Skills.
Knowledge, Skills and Abilities
Possess strong supervision skills, ability to communicate orally and written; ability to establish and maintain effective working relationships with individuals, community, and staff; ability to analyze findings, identify problems and formulate solutions; knowledge of Intellectual disabilities and related physical, health, and behavioral concerns; possess caring and positive attitude; experience in conducting comprehensive assessment and knowledge of documentation tools. Residential Coordinator must serve as QDDP and must also possess reliable transportation, a valid driver's license, and proof of auto insurance. This position requires the successful completion of training/certification in CPR, first aid, behavior management, medication administration.
