Job Summary:
The Providence Center Residential Case Manager Intern will assist the clinical case management services in order to support program goals and objectives using a quality care approach.
The Residential Case Manager Intern will help to identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders.
Support the clinical team and consumers to achieve the highest level of independence possible.
Duties and Responsibilities:
Support case management and support to consumers.
Treat consumers with respect.
Assist in providing support counseling, problem solving, contracts and limit setting.
Help to establish attainable goals with the consumer.
Collaborate with consumer, agencies and others in treatment planning.
Support the clinical team on engagement insight and needed treatment.
Provide support counseling with mental health issues, substance use disorders, and medical conditions.
Support the team assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment.
Help teach consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments.
Work with clinical team to initiate treatment and advocate for consumer within network of care.
Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
Support and complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements and abide by TPC confidentiality policy.
Maintain cooperative relationships with The Providence Center staff, consumers, community agencies and the public.
Attend trainings, case presentations and conferences as required.
Serve on appropriate Center committees.
Work flexible hours, including evenings, weekends, holidays and during inclement weather.
Requirements:
High School Diploma/GED and/or five years of experience in a behavioral health setting.
Experience in residential facilities is preferred.
Must have valid driver's license and insured auto.
Bilingual Spanish preferred.
Must be able to work flexible hours including evenings, overnights, weekends, and holidays as well as cover the program when unforeseen circumstances arise to ensure 24-hour coverage.
