About the Job
Essentials: + Provide advocacy, information and referral and case management to MPHA residents to enable them to live as independently as possible
+ Together with supervisor, evaluate and adjust services in the buildings as needed to ensure best outcomes for clients and residents
+ Outreach to individuals and groups within assigned buildings to ensure that residents and potential clients are aware of available services
+ Interpret and promote other VOA services and community programs to residents so that they are knowledgeable about resources available to them
+ Provide group services to residents to meet identified needs
+ Maintain case file documentation to ensure that professional standards of documentation are followed, and maintain client medical information, financial records and personal information in a confidential manner
+ Collaborate with other VOA services and community professionals to ensure that a wide network of resources is available for Highrise clients
+ Participate in case review sessions on a regular basis to help guide and strengthen social service skills
+ Participate in training opportunities to maintain and improve professional social service skills
+ Assures client privacy is maintained by being knowledgeable and following HIPAA privacy and security regulations
+ Work as many hours as needed to complete the job
