Position Responsibilities: The Resident Service Coordinator is responsible for the direct coordination of services and supports to residents at assigned housing site(s). Responsibilities include identifying and accessing resources beneficial to residents as needed, acting as a liaison between the Housing Authority and residents when appropriate, and working collaboratively with Housing Authority staff on behalf of all residents.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
- Coordinate and facilitate services/activities and schedule office hours at designated housing site(s).
- Inform residents about State Home Care and other publicly funded programs and public benefits. Make referrals and assist residents with securing benefits as appropriate.
- Recruit, train and supervise volunteers who will assist with group/congregate activities.
- Identify gaps in services for residents. Develop, implement and offer a range of services and programs to residents with the goals of establishing and maintaining cost effectiveness and affordable services, and meeting unmet needs.
- Assist in screening residents for Home Care Program eligibility and establish appropriate care plans with the AgeSpan Care Manager(s) and Nurse(s) assigned to the sites.
- Work closely with community organizations and social service providers to provide programs, supports and services to allow residents to age in place.
- Provide accurate and timely documentation to meet all contractual and administrative standards as required by the Department of Housing and Community Development, Elder Services and, if appropriate, the Housing Authority. Provide regular updates, feedback and reports to Housing Authority staff to preserve residents' tenancy.
- Organize regular meetings with residents to address social, recreational and educational needs. Identify problems and issues and approaches to dealing with them.
- Attend regularly scheduled meetings with supervisor and RSC team, and AgeSpan all staff meetings.
- Attend relevant trainings and workshops.
- Other related duties as assigned.
Qualifications: BA/BS Degree preferred; LSW preferred. Knowledge of and experience with housing/social services with elderly/disabled population strongly preferred. Knowledge of Housing/Disability laws and regulations helpful. Solid written, verbal, interpersonal and team skills. Strong computer skills and comfort with Microsoft applications and programs. MARSCH (Massachusetts Association of Resident Service Coordinators) Certification helpful but not required. SHINE (Serving the Health Insurance Needs of Everyone) certification a plus.
Hours: 37.5 per week
