Rate of Pay: $30-$36/hour

What You'll Do:
Resident Care Coordinator | RCM
- Manage care giving staff including hiring, training, evaluations and performance management
- Develop and post schedule ensuring adequate daily staffing. Communicate with staff regarding meetings, trainings, etc.
- Be available for after-hours staffing concerns
- Coordinate care team orientation and trainings with HR Manager
- Assist in coordinating the daily administrative and care giving functions of the Community under the direction of the Executive Director
- Organize supplies and materials, prepare work orders for maintenance department as required
- Manage care team for completion of housekeeping duties
- Assist nursing staff in communication with residents, families, physicians, and other outside healthcare providers
- Assist with the maintenance of resident records
- Supervise the personal care of residents and assist in monitoring the wellbeing and the environment of the residents
- Review and understand all resident individual service plans. Assist in ensuring that resident service plans accurately reflect resident care needs and are updated as needed
- Establish and maintain positive and professional working relationships with residents, and all team members
- Maintain knowledge of current OSHA and state regulations and routinely monitors Community compliance with the regulations related to resident care and nursing functions
- Observes residents for changes in physical, emotional, mental, and social condition, unusual symptoms, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurse or Resident Coordinator
- Completes all forms such as incident reports, and written and computerized documentation, as necessary for communication of
resident information with other shifts, Resident Coordinators, and with the licensed nurses - Ensure that changes in resident condition are monitored, documented, effective interventions are implemented, and reported appropriately to nurse’s physician and/ or family members
Resident Care Coordinator | RCM
- CNA, or previous experience working with the elderly in a residential or long-term care setting preferred
- Current CNA license, other license, or ability to obtain as required by State regulatory agencies
- Must enjoy working with seniors
- Ability to establish effective relationships with residents, family members and team members
- Must be able to read, comprehend, write, and speak English to communicate effectively with residents, families, other team members and physicians
- Must be able to communicate effectively with residents, families, team members, visitors, physicians, etc.
- Strong documentation skills
- Strong interpersonal skills
- Must have basic computer skills, including Word, Excel and Outlook
- Ability to develop systems for monitoring resident health conditions and for ensuring efficient and safe delivery of resident care
- Strong organizational skills and ability to re-prioritize daily tasks to accommodate fluctuating needs of the residents and the community
- Ability to represent the community in a positive and professional manner
- Must be able to meet all health-related requirements pertaining to state licensure
- Must maintain current CPR/First Aid and Food Handlers credentials
- Must maintain annual Continuing Education Credits
