Seeking a registered dietician to serve as one of two patient navigators for an NIH funded study of a navigator program for patients with diabetic foot ulcers. KEY RESPONSIBILITIES: + Functions as a member of a multi-disciplinary team that manages the care of the diabetic patient population, requiring a high degree of specialization in nutritional skills and knowledge. + Provides nutritional care to patients including assessment, counseling, education, evaluation and management. + Develops, conducts, and evaluates patient and family education by discussing the plan of care. + Determines appropriate interventions for identified patient problems, symptoms, behavioral changes and teaching needs. + Documents all aspects of patient care. + Communicates effectively with patient, family and members of the health care team. + Administers clinic policies and procedures in accordance with state, federal, and organizational guidelines at each institution; may assist in reviewing policies and procedures. + Develops and prepares operational and statistical reports for management and regulatory agencies to assist in making fact-based decisions and to comply with regulations. + Determines and mobilizes equipment and resources required for effective patient care. + Serves on Clinic committees. + May supervise support staff. + Performs related responsibilities as required. JOB DETAILS: + Conduct telephone calls and in person visits with enrolled patients to provide education and support according to the study protocol. + Coordinate and provide logistics support for patient appointments related to the trial. + Inform and coordinate available resources at Grady and within the community to support patients' needs. + Respond to participant inquiries and support calls during designated hours for study participants. MINIMUM QUALIFICATIONS: + Licensed dietitian or registered nurse in the state of Georgia. + Certified as a Diabetes Educator. + Two years of related experience. PREFERRED QUALIFICATIONS: + Registered dietician. + Candidates fluent in Spanish are highly encouraged to apply. + Certification as a Diabetes Educator (CDE) as well as experience in counseling on diabetes management are a plus. + Eligible for work without sponsorship in the US or hold an active Visa that can be transferred will be considered. NOTE: Position tasksaregenerallyrequired tobe performed in-person at an Emory University location.
