Job Details
Remotely case manage MLTC medicare advantage members.
Acting as a patient advocate to facilitate transitions of care
Collaborating with inpatient facilities and case management staff
Conducting telephonic outreach and discharge planning evaluations
Providing support for home health services
Utilizing critical thinking skills for effective problem-solving
Ensuring compliance with laws, regulations, and agency policies
Demonstrating proficiency in interviewing and physical assessment
Skills You Need:
Associates in Nursing; Baccalaureate Degree preferred
Minimum one year of acute, Medical-Surgical nursing experience
Discharge Planning/Utilization Management experience preferred
Proficiency in Word and Excel; current CPR certification
