Polaris Group, an Assembly Health company, is looking for a Remote MDS Coordinators who will partner with our organization to meet our objectives and help manage our client partnerships. This is a full-time, non-exempt position reporting to our Director of Consulting Services.
What our MDS Coordinators do:
- Participate in patient assessments by overseeing assessment process, setting schedules, and assuring accuracy.
- Determine accurate expenses associated with a potential admission.
- Review all applications for admission and communicate results of the assessment.
- Complete and assure the accuracy of the MDS process for all residents.
- Maintain current working knowledge of criteria, serving as a resource for staff and communicating changes in regulations.
- Look for potential risks and/or changes that may affect reimbursement.
- Facilitate problem-solving for complicated admissions.
- Start Medicare process and monitor assessment schedules and nursing documentation to ensure accuracy and timely submission.
- Maintain client confidentiality and function within HIPAA regulations.
- All other assignments and projects as directed by leadership.
What it takes to join the family:
- Experience in MDS coordinator role or related experience.
- Current and valid Compact nursing license.
- Valid and current RAC-CT.
- Experience in a clinical and healthcare setting. Deep medical terminology knowledge.
- Superb decision-making and problem-solving skills.
- Superior attention to detail.
- Integrity to maintain confidentiality when working with sensitive data.
- Adaptable to change and willingness to learn different processes.
- Ability to function well in a fast-paced and at times stressful environment.
- Prolonged periods of sitting at a desk and working at a computer. Ability to lift and carry items weighing up to 10 pounds at times.
