Position Overview:
We are looking for a motivated and versatile Hybrid Rehabilitation Supervisor/Field Physical Therapist to join our team. This dual role involves supervising our rehabilitation services and directly providing field physical therapy services to our patients.
Responsibilities:
- Oversee the clinical operations of our rehabilitation team, including physical, occupational, and speech therapists.
- Mentor and support rehabilitation staff, fostering professional development and clinical excellence.
- Ensure adherence to all regulatory compliance requirements and implement quality improvement measures.
- Collaborate with an interdisciplinary team to integrate rehabilitation services with overall patient care plans.
- Conduct patient assessments to determine physical condition and establish therapy goals.
- Implement physical therapy interventions based on individual patient needs and monitor progress.
- Educate patients and families on treatment plans and home exercises.
- Complete all required clinical documentation promptly and accurately.
- Monitor the effectiveness of therapy interventions and make adjustments as necessary.
- Ensure the delivery of high-quality care and meet therapy outcomes.
- Work closely with healthcare professionals including nurses, physicians, and other therapists to ensure a cohesive approach to patient care.
- Participate in case conferences and contribute to care planning discussions.
- Maintain compliance with federal, state, and local regulations and professional standards.
Qualifications:
- Bachelor’s or Master’s degree in Physical Therapy.
- Valid state licensure as a Physical Therapist.
- Proven experience in physical therapy
- Excellent communication and interpersonal skills, capable of collaborating in a multidisciplinary team.
- Ability to manage time effectively between supervisory duties and direct patient care.
- Familiarity and experience with OASIS and Wellsky (EMR)
