About the Job
A Long Island-based support services organization is looking to add a new, licensed Registered Nurse (RN) to one of their facilities in Queens County. In this role, the Registered Nurse (RN) will be responsible for providing health services to individuals in an IRA setting.
About the Opportunity:
- Setting: Residential facility
- Assignment Length: 3 to 6 months (contract to perm)
- Schedule: Monday to Friday
- Hours: 7am to 3pm, 8am to 4pm, 9am to 5pm, or 10am to 6pm
- Patient Population: Adult ID / DD ; Medically Fragile
- Caseload: 5 to 8 people per group home
Responsibilities:
The Registered Nurse (RN) will:
- Provide nursing care and treatment for emergency and minor malady relief
- Attend pertinent medical and psychiatric appointments to advocate for the individuals and ensure appropriate treatment plans
- Follow up on all treatment recommendations in a timely manner
- Ensure all medical and psychiatric treatment plans are compliant with current Standards of Care and State Regulations
- Check monthly medication order for accuracy and monitor for adequate supply of medication in the residence at all times
- Verify each newly prescribed medication and ensure that each is filled in a timely manner
- Communicate with families regarding medical issues, as necessary
- Provide nursing coverage to residences during assigned RN's PTO including on call telephone triage and site visits
- Obtain and maintain medical histories and records to assure continuity of each Individual's medical history in compliance with regulatory and legal requirements
- Participate as a member of the program planning process (attending and participating in ISP meetings)
- Assist in the interview process of candidates for the medical support counselor position
- Perform other duties as assigned
Qualifications:
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
- Solid assessment, clinical, and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- Bachelor's Degree in Nursing
- Experience with the Intellectual / Developmental Disabled population
