Job Responsibilities
- Receiving orders and care plans from a virtual care team for patients during their acute phase of illness
- Working with virtual nurses and hospitalists to identify interventions and other needs for patients in the hospital at home setting
- Providing comprehensive daily patient assessments and ongoing support for advanced care planning
- Administering vascular access care, wound care, infusion, and other nursing interventions
- Providing patients with health education and coaching regarding treatment decisions
- Making evidence-based clinical decisions that align with scope of practice, medical necessity guidelines, practice standards, industry standards, best practices, contractual requirements, and legal directives
- Leading clinical operations and providing medical management across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating)
- Participating in home visits and transition of care management for patients
- Obtaining, documenting, and reporting information about specific symptoms and questions
- Promoting patient engagement through empathetic care that develops rapport and builds trusting relationships
Qualifications and Skills
- Active Connecticut state Registered Nurse (RN) license
- 2 years’ experience in an acute care, in-patient, hospital setting, with 1-3 years of recent experience
- Ability to place a peripheral line with experience maintaining and accessing PICCs and midlines
- CPR for Healthcare Providers Certification (from the American Heart Association)
- Ability to travel to multiple patients' homes during shifts
- Strong clinical and assessment skills and the ability to work independently
- Hepatitis B and MMR records, with current influenza vaccination
