RN-Primary Care Nurse-Full Time

Parker Jewish Institute Hospice and Home Health Care is seeking full-time RN-Primary Care Nurses to join our Hospice and Home Care division.

This position requires the full understanding and active participation in fulfilling the mission of Parker Hospice and Home Care is to deliver exceptional care and to enhance the quality of life of our patients.

The Primary Care Nurse is responsible for the direct care and documented coordination of patients of the Hospice and Home Care programs. Under the direction of the Nursing Supervisor, the Primary Care Nurse assures patient eligibility to continue on the program.

Key Tasks:

• Provides professional skilled nursing service in patient's home, based on theoretical nursing concepts of practice and the Policy and Procedure of the CHHA
• Promotes and maintains the health of patients in the community through teaching, counseling, and providing direct nursing care
• Assures compliance with Federal and State regulations
• Oversees patients in the community by offering professional supervision, health teaching, and hands on care in order to maintain patient safety in their home environment
• Ensures patient information is kept current in Allscripts

Required Qualifications:

• Current New York State license as a registered nurse (RN)
• Baccalaureate degree in nursing from an accredited school preferred
• One-year Medical/Surgical experience and/or previous home care or hospice experience preferred
• Knowledge of care coordination a plus
• Must be able to use computer technology in the field
