Registered Nurse (RN) Care Manager - CHN Healthcare Group

CHN is currently assisting a top health plan in the greater Boston area with a need for a RN Care Manager for their SCO plan.




This position is remote based (work form home) and will require visits with members to conduct in person assessments when needed. Travel territory coverage would include: Woburn, Lexington, Medford, and Malden.




Full-time, Monday-Friday (40 hours/week).




No weekends, holidays, or on-call required.




Responsibilities:

-Requires the ability to travel to member's homes, hospitals, and skilled nursing facilities.

-Collaborate with other healthcare professionals to coordinate patient care and ensure effective communication.

-Timely completion of initial and ongoing geriatric assessments

-Completion of the Minimum Data Set-Home Care (MDS-HC)

-Proactive management and follow-up via home visits and telephone according to the member's care plan.




Skills:

- Valid RN license in the state of MA

- Proven experience in home care (home health), insurance, critical care, behavioral health, or some combination thereof.

- Excellent communication and interpersonal skills to foster collaboration among healthcare professionals.

-Sound clinical judgment and decision-making abilities.

- Ability to prioritize tasks and manage time effectively.

- Knowledge of insurance and community services preferred

Salary: $86,000.00 - $92,000.00 per year
