Registered Nurse (RN) - Adult OPWDD - Tandym

A Long Island-based nonprofit healthcare organization is looking to add a licensed Registered Nurse (RN) at one of their residential facilities in Suffolk County.

About the Opportunity:

• Setting: Residential facility
• Schedule: Monday to Friday
• Hours: 7:30am to 4pm or 10am to 6:30pm
• Patient Population: Adult ID/DD OPWDD
• Patient Volume: Caseload of 20
• EMR: Epic

Responsibilities:

The Registered Nurse (RN) will:

• Serve as the medical liaison for IDT team and ensure proper medical follow-up
• Assess injuries/accidents to ensure individuals receive proper medical care
• Develop written plans for each person; modify nursing plan, as necessary
• Maintain medical records for each individual
• Accompany individuals to medical appointments, as assigned
• Ensure proper medical follow-up through an on-call system
• Ensures all medication is administered and stored, as necessary
• Complete appropriate incident reporting paperwork, as required
• Act as a liaison with family members regarding medical issues
• Provide ongoing training and supervision of AMAP certified staff
• Perform other duties, as needed

Qualifications:

• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN) license
• Solid assessment, clinical, and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

Desired Skills:

• Bachelor's Degree in Nursing
• OPWDD background
