Job Details
Patient Care and Evaluation:
- Perform thorough health evaluations, including gathering medical histories and measuring vital signs.
- Assess and prioritize patient needs based on their presenting conditions.
- Collaborate with physicians and healthcare teams to create and execute personalized care plans.
Care Coordination:
- Facilitate patient care across various healthcare services and settings.
- Manage patients with chronic conditions through education, monitoring, and follow-up care.
- Assist with transitions of care and referrals to specialists or community resources.
Patient Education:
- Educate patients and families about health issues, treatment options, and preventive measures.
- Provide guidance on medication adherence, lifestyle changes, and self-management techniques.
Clinical Procedures:
- Administer prescribed medications and vaccinations.
- Conduct routine clinical tasks such as wound care, injections, and basic diagnostic assessments.
- Support minor procedures as required.
Documentation and Quality Assurance:
- Maintain precise and timely electronic health records.
- Engage in quality improvement projects and help establish best practices.
- Ensure adherence to regulatory standards and clinic protocols.
Qualifications:
- Current Registered Nurse (RN) license in the state.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) considered.
- At least 1-2 years of nursing experience, ideally in primary care or community health settings.
- Basic Life Support (BLS) certification required.
- Strong clinical assessment and analytical skills.
- Excellent communication and interpersonal abilities.
- Familiarity with electronic health record (EHR) systems.
- Commitment to serving diverse and underserved populations.
