Registered Nurse Navigator - Cayuga Health System

The Oncology Nurse Navigator is a professional registered nurse with oncology specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. She/he serves as a consistent care coordinator ensuring timely access and seamless coordination of services throughout the continuum of oncology care. The nurse functions as a part of the multidisciplinary team providing medical information, education, and shares knowledge regarding internal and external support services and resources.

Roles and Responsibilities:

• Advocate for the health and well-being of patients, families, and colleagues, ensuring their needs and concerns are addressed.
• Influence positive patient outcomes and promote positive system outcomes through improved interdisciplinary communication, patient retention, and downstream revenue.
• Assess barriers to care and engage patients and families in creating solutions to financial, practical, and social challenges.
• Facilitate and advocate for patients with physicians by helping patients understand and comply with referring provider instructions.
• Educate patients and caregivers on the multidisciplinary nature of treatment, roles of team members, and what to expect from the healthcare system.
• Ensure continuity of care and support patient self-management goals through collaboration and communication with the patient's physicians, and evaluate clinical and quality outcomes for oncology patients.
• Serve as a liaison for patients, families, physicians, and other healthcare providers, maintaining positive relationships and ensuring effective communication, documentation, and coordination with referring physician offices and pharmacies.

Required Skills and Experience:

• Graduate of an accredited school of nursing
• Active NYS Registered Nurse Licensure

Preferred Skills and Experience:

• Bachelor Degree in Nursing
• Three years as a Registered Nurse

Physical Requirements:

• Lifting up to 20 lbs., standing or sitting for lengths of time. Repetitive use of hands and fingers (e.g., typing, use of computer and keyboard).

Location and Travel Requirements:

• Onsite

Pay Range Disclosure:

• $35.50 to $44.00 per hour
