- The ideal candidate should have be experienced in the Hospice Industry, communication skills, and the ability to work independently and as part of a team.

Key Responsibilities:
- Coordinate and provide care that is safe, timely, effective, efficient, and client-centered
- Handles and manages case assignments
- Facilitate multiple care aspects (case coordination, information sharing, etc.)
- Aid patients in making informed decisions by acting as their advocate
- Develop effective working relations and cooperate with the clinical team throughout the case management process
- Adhere to professional standards as outline by protocols, rules and regulations
- Completing initial, comprehensive, and ongoing assessments for patients and families to determine hospice needs
