About the Job
RN Care Manager Compensation:
Pay: $36 - $42 per hour based on experience, plus mileage reimbursement.
Benefits: Available to full-time employees after 90 days of employment.
401(k): Company match available for full-time employees with 1 year of service on eligibility dates.
RN Care Manager Qualifications:
Active RN license in the state of Oklahoma.
Experience with Pediatrics required.
Preferred Experience: Care/Case Management and Electronic Medical Record (EMR) experience.
Strong computer and documentation skills.
RN Care Manager Responsibilities:
Monitor email and voicemail for important communications and member needs.
Communicate effectively with providers as necessary.
Address member needs promptly and effectively.
Plan and schedule in-person visits with members.
Conduct required monthly telephonic outreach to members.
Complete all necessary documentation in the EMR system.
Conduct biannual assessments and required documentation during home visits.
Attend mandatory meetings as scheduled.
Collaborate with teams to resolve member issues.
