Summary:
The Institute for Family Health (Institute) is a non-profit organization committed to providing health services to medically underserved communities. The Institute operates 30 full and part time health centers throughout the Bronx, Manhattan and the mid-Hudson Valley region of New York State. The Co-Care study Nurse Care Manager (NCM) is part of a collaborative care model for a randomized clinical trial which aims to examine the efficacy of a primary care intervention for patients with polysubstance use. Key duties of the NCM include partnering with PCPs in caring for patients with polysubstance use. The NCM provides direct, individual care to patients of enrolled providers in the trial. The NCM role centers on establishing collaborative relationships with people at all levels of care: the patient, the clinic staff, and the PCPs. Creating effective working relationships with all these people is critical to the smooth implementation of the intervention. This position is based at two Institute primary care practices in the Bronx.
Principle Duties and Responsibilities (75%)
- Conduct comprehensive nursing assessment, including social, medical, mental health, substance use, and medication history.
- Provide ongoing care management and chronic disease management, education, and support for patients.
- Work with the PCP to manage substance use disorder (SUD) medications and facilitate the contingency management program for intervention patient participants with stimulant use disorder (StUD)
- Complete timely and appropriate documentation and record keeping in the electronic health record
- Population management, including proactive outreach, use of a clinical registry, and regularly reviewing cases with the addiction specialist (a consultant who supports the NCM and PCP in formulating treatment plans and adapting them to optimize treatment response and meet the needs of individual patients)
- Provide intervention patient participants with health education and counseling on risk reduction, overdose prevention, and self-management skills
- Provision of care and advocacy for patients of all ages and their families based on prudent nursing judgment and acceptable standards of practice.
- Perform tasks and procedures within the scope of a registered nurse (R.N.) and legal guidelines of the Nurse Practice Act.
- Involve patients in the development of the treatment plan, and working with the patients' needs
- Ability to practice trauma-informed care and apply de-escalation techniques
- Identify community resources and promote access or referral to them
- Maintain patient confidentiality in alignment with HIPAA
- Return phone calls, emails and other forms of communication in regards to trial and routing questions to appropriate prescriber as needed.
- Establishes and maintains positive relationships with participants and study site affiliates.
- Demonstrates a commitment to foster inclusiveness and diversity though interactions and communications with others.
Miscellaneous (10%)
- Participates in on-going trainings and keeps abreast of developments in the field.
- Performs other duties as assigned.
EDUCATION
Bachelor degree (or 3 years related experience) - Preferred
