Registered Nurse Assessor - Tandym

A home health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their team in Queens as a Nurse Assessor.

Responsibilities:

The Nurse Assessor (RN) will:

• Meet one on one with the person in the adult home to complete the clinical assessments
• Maintain scheduled assessment appointments
• Provide clinical expertise to determine class members' eligibility
• Advocate and recommend additional clinical resources to ensure that the client remains successfully housed in the community
• Perform other duties, as needed

Qualifications:

• 5+ years of experience in a Mental Health role/setting
• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN) license
• Experience completing Psychiatric Assessments
• Knowledge of the Community Mental Health System and various community resources
• Solid understanding of Public Entitlements and Enrollment requirements related to Managed Long-Term Care and Certified Home Health Agency services
• Solid assessment, clinical, and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

Desired Skills:

• Bachelor's Degree in Nursing
• Has experience with geriatric patient care
