We are seeking a dedicated and experienced Long Term Care Dietitian to join our team at Apex Nursing and Rehabilitation Center. The ideal candidate will be responsible for providing nutrition care to our long-term care residents, ensuring that their dietary needs are met in accordance with their health conditions and personal preferences. This role involves developing and implementing individualized nutrition plans, collaborating with healthcare professionals, and promoting overall wellness through nutrition education and support.
Responsibilities:
- Nutrition Assessment and Care Planning: Conduct comprehensive nutritional assessments for new and existing residents. Develop, implement, and regularly update individualized nutrition care plans based on resident needs, preferences, and medical conditions.
- Menu Planning and Review: Collaborate with the food service team to create and review menus that meet the dietary needs of residents while adhering to regulatory standards. Ensure menus are nutritious, balanced, and suitable for various dietary restrictions.
- Resident Education: Provide education and counseling to residents and their families on nutrition-related topics, including dietary modifications, special diets, and healthy eating practices.
- Collaboration: Work closely with physicians, nurses, and other healthcare professionals to coordinate care and address any nutrition-related issues or concerns.
- Compliance: Ensure that all dietary practices comply with state and federal regulations, as well as facility policies. Maintain accurate and confidential records of nutritional assessments and interventions.
- Quality Improvement: Participate in quality improvement initiatives related to dietary services. Monitor and evaluate the effectiveness of nutrition care plans and make adjustments as needed.
- Training and Support: Provide training and guidance to dietary staff and assist in the development of ongoing educational programs related to nutrition and dietary services.
Qualifications:
- Bachelor’s degree in Dietetics, Nutrition, or a related field.
- Registered Dietitian Nutritionist (RDN) credential required.
- Minimum of 1 year of experience in clinical nutrition, preferably in a long-term care setting.
- Strong knowledge of therapeutic diets, food service operations, and regulatory requirements.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work both independently and as part of a multidisciplinary team.
- Compassionate and empathetic approach to resident care.
