The Regional Social Services Director supports residents across skilled nursing facilities by identifying psychosocial, mental, and emotional needs and connecting them with appropriate services. This role ensures strong communication between residents, families, and care teams while overseeing social services and activity programs.

Key Responsibilities:

• Provide direct social services to residents and families
• Assist in accessing facility and community-based resources
• Strengthen communication between residents, families, and staff
• Collaborate with care teams to meet residents’ needs
• Oversee Social Services and activity programs
