REENTRY SPECIALIST POSITION SUMMARY:
The Reentry Specialist provides access to community programs, ensure continuity of care following release into the community, advocate for patient needs, and act as liaisons between the multiple systems impacting the lives of offenders with mental health illnesses, co-occurring substance use disorders, and chronic medical illnesses.
REENTRY SPECIALIST MINIMUM REQUIREMENTS:
- One year of experience in the social services field.
- A bachelor's degree in Mental Health or closely related field.
- Case Management experience
- Applicants with experience in a correctional health care setting will be given preference.
