As a member of the PACE Interdisciplinary team, the Recreation Coordinator assesses the needs, interests and capabilities of participants and develops individualized therapeutic recreation plans. In addition, plans, directs and participates in group and individualized recreation programs at the PACE Center in collaboration with Recreation Assistants.
Under the supervision of the Program Manager, works with the Recreation Assistants in providing recreational services and Activities of Daily Living (ADL) assistance to participants attending the PACE Center.
Responsibilities:Recreational Activity Planning and Implementation
• Plans and implements group and individual activities to improve or maintain the participants’ cognitive, physical, social, psychosocial, spiritual and artistic needs.
• Plans and implements activities for dementia-specific population. Oversees daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming and sensory stimulation.
• Organizes community outings and special events at the PACE Center. Duties include: arranging for participant transportation, entertainment, catering and involving other PACE staff.
• Develops a monthly calendar of activities collaboratively with Lead Rec Assistant and Recreation Assistant.
• Schedules and evaluates monthly program plans to promote effective activities & events according to participant abilities and preferences within the PACE Center daily flow of events.
• Maintains general order in the activities area, including ensuring daily clean-up and routine craft/supply ordering.
• Assists Program Manager with ordering recreational/craft supply ordering.
• Assists Program Manager with PACE Volunteer oversight and training.
• Completes risk reports per company policy.
• Fosters team building and professional relationship amongst staff.
• Assists in the safe supervision of Participants at the PACE Center and while out in the community for special events/outings.
• Adheres to PACE Participant Rights at all times.
PACE IDT Member
• Functions as a member of the interdisciplinary team, participating in IDT and Plan of Care Discussions.
• Reports changes in participant’s baseline status to appropriate staff as needed.
• Completes timely documentation including initial, semi-annual, annual and change in status assessments and care plans; documentation on individual recreation plans/interventions; and other information necessary for quality PPT care, such as documenting PPT behaviors and responses to interventions.
• Assists participants and caregivers by providing leisure education for activity modification or skill development, and supplies for in-home use, as needed.
• Contacts assisted living homes and nursing homes regarding participants that have been placed. Assessment includes participant involvement and new ways to enhance activity/recreation.
• Assist with competency and orientation for new team members.
Personal Care/ADLS
• Assists participants with personal care, toileting, showers, grooming, cueing and feeding as needed*. May involve use of transfer equipment (*CNA certification required)
• Assists with laundry service done at the PACE Center.
• Ensures oxygen concentrators and tubing are available for oxygen dependent participants attending the PACE Center.
• Assists participants to and from activity areas within the PACE Center including pushing PPTs in wheelchairs and assisting PPTs in ambulating with walkers and gait belt as needed.
• Assist with serving meal/snacks to participants in accordance with dietary and food service regulations.
Basic UPH Performance Criteria

• Demonstrates ability to meet business needs of department with regular, reliable attendance.
• Employee maintains current licenses and/or certifications required for the position.
• Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
• Completes all annual education and competency requirements within the calendar year.
• Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.
Education
• High school diploma and specific training in geriatrics, including dementia, required.
• Successful completion of Certified Nursing Assistant Program
License(s)/Certification(s)
• Valid driver’s license and proof of auto insurance.
• Maintains current BLS status
• Maintains current Adult & Child Mandatory Abuse Reporter status
• Completes 12 hours of related in-service education per calendar year.
Knowledge/Skills/Abilities
• Working knowledge of recreational therapy techniques and practices of the elderly including assessment, care plan development, monitoring and evaluation needs.
• Working knowledge of physical, psychosocial and behavioral needs of the elderly population.
• Excellent oral and written communication skills.
• Demonstrated creative abilities to plan, develop, and implement varied activities to meet diversified needs.
• Ability and experience working with volunteers.
• Good working knowledge of dementia disease process.
• Familiarity with illnesses common to frail older adult population.
• Basic computer skills.
