About the Job
The Quality Manager, Ambulatory Care is responsible for care coordination in the achievement of improved preventive and chronic care measures. Develops, implements, monitors, and coordinates quality improvement programs that support the goal of improving performance for HEDIS/QARR and other CMS/DOH programs. Identifies program resources, establishing workflows, developing actionable and ongoing reports with meaningful goals, and providing analysis of reports on where actions should focus to address quality improvement deficiencies. Supervises employees making outreach calls, completing chart reviews, and providing care management. Works with providers, care managers, patient navigators, Medical Office Assistants in planning and implementing workflows that addresses preventive and chronic care goals. Evaluates and presents on progress of ambulatory care quality goals.
* Bachelor’s Degree
* Minimum of two (2) years of experience in an ambulatory care and clinical setting
* Minimum of two (2 years of managerial and/or project management experience.
* Knowledge of PCMH, HEIDIS, and MIPS programs.
* Ability to manage multiple priorities
* Ability to lead and be a change agent
* Excellent customer service skills, responsible for meeting deadline, punctual,.
* Effective written and verbal skills
* Ability to maintain confidentiality.
* Advanced reporting and analysis skills
* Keep commitments, and when unable, gives notice.
